Vote-by-Mail ballots are NOT FORWARDABLE MAIL!!!
Vote-by-Mail is a service provided for registered voters who wish to vote in the comfort of their homes or for those who may be away from Putnam County during the election.
A mail ballot may be requested for either a specific election or for all elections through the end of the election cycle, or through one general election..
Vote-by-Mail Ballots may be requested:

Only the voter, or a designated member of his or her IMMEDIATE family or legal guardian can request a vote-by-mail ballot for the voter.
Immediate family means the voter's spouse or parent, child, grandparent, grandchild, or sibling of the voter or the voter's spouse.
A request for a Vote-by-Mail ballot from the voter must include the following information:
- Voter's name
- Voter's residential and mailing address
- Voter's date of birth
- Voter's FL driver license number, or FL identification card number, or the last four digits of the voter's social security number
- Voter's signature; if the request is in writing
A request for a Vote-by-Mail ballot from a designated IMMEDIATE family member or legal guardian must include the voter information as listed above in addition to the following information:
- The requester's name
- The requester's address
- The requester's driver license number, identification number, or the last four digits of the requester's social security number
- The requester's relationship to the voter
- The requester's signature (written requests only).
- The designee will need to provide their driver's license, identification card, or other form of picture identification

| Update Address Residence/Mailing | Vote-by-Mail ballots are NOT FORWARDABLE!!! Update your address to prevent any delivery delays. RETURNED MAIL will VOID your Vote-by-Mail ballot request. |
| Other Mailing Address | A request for a ballot to be mailed to an address that is NOT currently on your Voter Record MUST be made in writing and signed by the voter. Contact the Elections Office for a Vote-by-Mail Request Form. When completing this form, be sure to include the alternate address in the "mail ballot to" section. |
| Last Day toRequest | All requests for ballots to be mailed MUST be received no later than 5 p.m. on the 12th day prior to an election. |
| Pickup by other than voter | A voter may provide a written authorization for a designee to pick up a Vote-by-Mail Ballot. This may be done after Vote-by-Mail ballots have been mailed out and up to 7 P.M. on Election Day. Contact the Elections Office to request an Authorization Form. |
| Signature Required | After marking your ballot and placing it in the envelope provided, seal the envelope, complete the Voter's Certificate on the back and PLEASE SIGN and date. |
| Forgot to Sign | If you submit a voted Vote-by-Mail ballot with an unsigned Voter's Certificate, a Vote-by-Mail Cure Affidavit must be submitted no later than 5 p.m. on the 2nd day after the election. You may obtain this affidavit from the Elections Office. Check the status of your Vote-by-Mail ballot. |
| Signature Differs | If you submit a voted Vote-by-Mail ballot the signature on the Voter's Certificate must match the signature on your voter voter registration record. If not, a Signature Cure Affidavit (English / Spanish) must be submitted no later than 5 p.m. on the 2nd day after the election. It is also suggested to complete a Voter Registration Application so your Voter Record can be updated with your current signature. Check the status of your Vote-by-Mail ballot. |

A Vote-by-Mail ballot may not be delivered to an elector or an elector's immediate family member on Election Day unless the voter affirms on an Election Day Vote-by-Mail Ballot Delivery Affidavit to an EMERGENCY that keeps the voter from being able to go to his/her polling place to vote per Section 101.62, F.S. |
